Your first visit involves a few special steps just so that we can get to know and understand you. We’ve made sure you can easily find us with map and directions on our website, the hours, online scheduling and more. Please download and fill out our NEW PATIENT PAPERWORK and bring it with you. We also want to describe your initial first visit so you can feel comfortable when you come in.
We will want to compile a medical history as that is essential in preparing treatment for you. These are some of the things we would like to know that are helpful. Have you ever experienced:
- Back problems
- Bone disorders (such as osteoporosis)
- Circulation problems
- Digestive problems
- Dizziness / vertigo
- Heart conditions
- Infections
- Injuries such as broken bones, sprains,
- Joint disorders
- Hernias
- Muscle problems
- Sleep problems
When problems such as those above, we’d need to know:
- When and how did your pain start?
- Where is the pain centered?
- Was the pain immediately following an injury?
- Is there anything you do that improves or worsens the pain?
We will do a physical exam along with a series of simple exercises. These exercise allow us to gather information about your motor skills, balance and gait. A short exam to explore the alignment of your spine is done as well. We need to discuss your lifestyle, nutrition and exercise to make sure to improve your healing and maintain health.
